residential withdrawal and rehabilitation program.

Who runs Triple Care Farm?

Martin Youth Foundation provides funding to support Triple Care Farm (Knights Hill, NSW) and the Youth Network aftercare service (four locations). These programs are run by Mission Australia.

When you apply to Triple Care Farm, Mission Australia will manage your application. Martin Youth Foundation cannot provide information about the application process or outcomes.

How much does treatment cost?

Treatment at Triple Care Farm costs $210 per week. If you receive Centrelink payments, you can use them to pay this fee.

Meet the
eligibility criteria

To be eligible for the program, you must:

  • meet the age requirements (no younger than 16, no older than 24)
  • use drugs, alcohol or multiple substances daily or almost daily
  • be physically, mentally and developmentally capable of participating
  • be committed to doing the program (you can’t be forced by someone else)
  • be an Australian resident or currently reside in Australia.

As part of the application process, you’ll be invited to attend an interview where you can tell the team more about yourself and your circumstances.

Apply for yourself or for a family member

Download and complete the Triple Care Farm Application Form.

You’ll need to provide:

  • contact and emergency contact details
  • family and background information
  • additional identification information (like a Medicare number)
  • information about medical and/or mental health conditions

Note: Programs at Triple Care Farm are voluntary. If you’re applying for a family member, you’ll need to have their permission.

Apply as a support worker, health worker or justice worker

If you’re referring a young person to Triple Care Farm, you’ll need to complete:

Note that if you’re referring from FACS, Mental Health, Department of Juvenile Justice, Probation and Parole, or Ageing, Disability and Home Care, or if the young person is supervised by these departments, you’ll need to provide the following additional documents:

  • Current comprehensive reports or court reports
  • Psychological reports completed during the last four years
  • Other relevant documentation that may assist their application

Contact the Triple Care Farm intake team

Once you’ve submitted your application, you’ll need to contact the Triple Care Farm Intake Coordinator within two weeks to talk through the next steps. (If you’ve applied on behalf of a young person, they’ll need to contact Triple Care Farm themselves).

  • If you’re eligible for the program, you’ll be invited to attend a video interview.
  • If you’re incarcerated, you’ll need to be granted day bail to complete this step.
  • If you’re approved for the program after interview, you’ll be put on the waitlist until a place becomes available.
